JOB SUMMARY

The case manager is primarily responsible for providing Case Management services on specified patient populations. This includes, but is not limited to: tracking utilization of resources, coordinating complex treatment protocols, cooperate with a multidisciplinary team to facilitate discharge planning, acting as a resource for external and internal customers and developing and implementing disease management programs. 

 

EDUCATION REQUIREMENTS & EXPERIENCE REQUIREMENTS

 (A = basics; B = preferred)

 

EDUCATION      

 

A.Registered Nurse, ADN

B.Certification in area of specialization, BSN

 

EXPERIENCE    

 

A. Five years nursing experience

B. Proven program development skills

 

LICENSES           

 

A. TX RN license, valid Texas Drivers license

B. Case Management Certification

 

SPECIAL SKILLS

 

A. Proven communication and problem solving skills; Computer literate

B. At least 2 years in area of specialization, 2 years of Case Management/Utilization Review experience

 

OTHER

 

A. Managed Care knowledge
