    JOB SUMMARY

    The Director of Population Health is responsible for contributing to the growth and profitability of Kelsey-Seybold Clinic by professionally managing services and activities for the organization’s Population Health Management Programs. This position is responsible for providing the oversight and direction for designing, implementing, and supporting a robust enterprise wide population health management program, that is aligned with the “Triple Aim”.

     

    The incumbent will be responsible for the development, implementation and ongoing review of the enterprise wide continuum of care and integrated case management system. This position will collaborate with and engage senior executive and physician leadership, and other staff throughout the organization to implement the population health program and promote a culture consistent with Kelsey-Seybold’s mission, core values, and standards of conduct. 

     

    EDUCATION REQUIREMENTS & EXPERIENCE REQUIREMENTS

    Education

    A.

    BA / BS required

    B.

    MBA or a master’s degree in Public Health Administration, Public Policy, Economics, or related fields highly preferred 

    Experience

    A.

    • 2+ years Population Health and Practice Transformation experience
    • 3+ years working with ACO's in clinical support programs
    • 8+ years of work experience which includes experience in cross-functional business areas such as general management, strategic planning, business development, project management and people management
    • 10 years in the Healthcare Industry and Operations

    B.

    • Critical thinker-this position will need to evaluate merits of new and innovative ideas and make sound decisions
    • Strong influencer- this position will partner with Clinical resources to enable practice transformation
    • Strong relationships-this role will work with Clinical Directors, Medical Directors, Sr. Leadership as well as business and technology partners to plan, direct and develop clinical programs
    • Strong Communicator-this role will need to be extremely comfortable presenting program objectives, scope, and process across segments
